Parasagittal cerebral injury is one of the major neuropathologic varieties of neonatal hypoxic-ischemic encephalopathy. The lesion involves the cerebral cortical and subcortical white matter with a characteristic location at the parasagittal, superomedial aspects of the cerebral convexity. One hemisphere may be more involved than the other, and the parietooccipital region is more frequently affected. The lesion has been diagnosed by pneumoencephalography, technetium brain scan, computed tomography, and recently positron emission tomography. We report the serial sonographic findings of parasagittal cerebral injury in an asphyxiated newborn that was confirmed by CT scan.
